The cave is amazing, and you get some little oil lamps to light the way, which is quite fun. But it’s a big touristic attraction, so perhaps you will need to wait in some lines. It’s still worth it.

Most likely not. Even in a car you'd have a tough time seeing all three in one day. Pick one and focus on that. IMHO given your limited time, I'd try to see Hohenwerfen castle.
